Vitiligo is a skin disorder marked by depigmentation, leading to the development of white patches on the skin. This occurs when the cells responsible for producing melanin, called melanocytes, are damaged or destroyed. Melanin is the pigment responsible for imparting color to the skin, hair, and eyes.

Vitiligo can develop at any age and on any part of the body, and the severity and extent of the depigmentation can vary from person to person. It is not contagious or life-threatening, but it can have significant social and psychological impacts on individuals affected by it. There is currently no cure for vitiligo, but various treatment options such as topical corticosteroids, phototherapy, and cosmetic camouflage may help manage the condition and improve the appearance of the affected skin.

Signs and Symptoms of Vitiligo

The signs and symptoms of vitiligo can vary from person to person, but some common indicators include:

  • White Patches: the presence of depigmented or white patches on the skin
  • Symmetrical Patterns: patches often develop symmetrically, appearing on corresponding areas on both sides of the body
  • Premature Graying of Hair: premature graying or whitening of hair on the scalp, eyebrows, eyelashes, and other body hair
  • Change in Eye Color: in some cases, a change in the color of the iris or the retina
  • Sensitivity to Sunlight: depigmented skin areas may be more susceptible to sunburn due to the lack of melanin's natural UV protection
  • Koebner Phenomenon: new patches developing at sites of skin injury or trauma

What Causes Vitiligo?

The precise cause of vitiligo remains not completely understood, but it is thought to result from a complex interplay of genetic, autoimmune, and environmental factors.

Types of Vitiligo

Vitiligo is classified based on the distribution and pattern of depigmentation. Non-segmental Vitiligo (NSV) includes Generalized Vitiligo, the most common type, where depigmentation occurs symmetrically on both sides of the body across areas like the face, hands, and limbs, and in rare cases, Universal Vitiligo, where depigmentation progresses to cover almost the entire body. Segmental Vitiligo (SV) occurs on one side or a specific segment of the body and often progresses for a period before stabilizing. Mixed Vitiligo shows characteristics of both non-segmental and segmental types. Focal Vitiligo presents as only a few scattered white patches, while Mucosal Vitiligo involves depigmentation of mucous membranes, such as the lips and the inside of the mouth.

Common Treatment Options

Treatment is personalized based on factors such as the extent of the condition, the patient's medical history, and individual preferences. Options may include topical steroids corticosteroid creams or ointments to reduce inflammation and promote repigmentation; phototherapy (PUVA or UVB) exposure to ultraviolet light, alone or combined with psoralen, to stimulate pigment production; topical calcineurin inhibitors such as tacrolimus or pimecrolimus, which modulate the immune response to encourage pigmentation; micro skin or camouflage makeup, which cosmetically conceals patches for a more uniform appearance; and excimer laser therapy, a targeted approach focused on repigmenting specific affected areas.
